What is a care report?
A care report is a structured summary of the care recipient's recent health data: current medications with dosages and schedules, recent vitals readings with trend analysis, visit observations from the care circle, behavior changes, and any concerns logged by family members. It gives healthcare providers a clear, organized picture of what has been happening between appointments. Doctors consistently report that this kind of documented information makes appointments far more productive than relying on memory alone.
Generating a report
Before a doctor visit, go to Care Hub and select "Generate Report." Choose the time period you want to cover: the last 7 days, 30 days, 60 days, or 90 days. Thirty days is typical for routine visits, while longer periods may be useful for specialist appointments. Select what sections to include. The report is generated as a clean, printable document with clear headings and organized data that a healthcare provider can scan quickly.
What the report includes
A typical care report contains: the current medication list with dosages, frequency, and prescribers; recent vitals readings with trends highlighted (such as "blood pressure averaged 145/88 over the past month, up from 132/80 the previous month"); visit log highlights and observations from caregivers; behavior and mood patterns; any new symptoms or concerns; weight changes; and a list of prepared questions for the provider. The system summarizes key observations into a concise narrative the doctor can read in under a minute.
Sharing and printing the report
You can print the report directly from the app, download it as a PDF, or email it to the doctor's office before the appointment. Having this information ready in advance means the doctor spends less time asking what has changed and more time addressing concerns. Many families find it helpful to email the report a day or two before the appointment so it can be added to the medical chart. The print-optimized layout is designed to be clean and professional.
Tips for better reports
Generate the report 1-2 days before the appointment so you have time to review it and add anything missing. The more consistently you track medications, vitals, and observations throughout the month, the more comprehensive and useful these reports become. Ask the doctor whether they prefer to receive the report by email beforehand or as a printed handout during the visit. Over time, the series of reports creates a longitudinal record of care that is invaluable for tracking disease progression and treatment response.
