Tracking Acute Health Events
Log falls, ER visits, infections, and other acute events to build a complete health picture.
What counts as an acute event
Acute health events are sudden, time-bound incidents that disrupt normal care routines. These include falls, emergency room visits, hospitalizations, infections (like UTIs or pneumonia), sudden changes in cognition or behavior, medication reactions, and similar episodes. They differ from chronic conditions, which are ongoing and managed over time. Tracking acute events separately helps you spot patterns, such as recurring falls or frequent infections, that might indicate an underlying issue worth discussing with the care team.
How to log an acute event
Navigate to Care Hub and select "Acute Events." Click "Log Event" and fill in the type of event, the date it occurred, a description of what happened, where it happened, and any immediate actions taken. You can also note which providers were involved, whether it led to hospitalization, and what follow-up was recommended. The more detail you include, the more useful the record becomes for future doctor visits and pattern analysis. You can add events retroactively if you did not log them at the time.
Recovery guides and post-event checklists
After you log certain types of events, Senior Support Resources may generate a recovery guide tailored to the situation. For example, after a fall, you might see a checklist covering follow-up appointments, home safety modifications to consider, exercises that may help with balance, and warning signs to watch for. After a hospitalization, the guide may cover medication reconciliation, follow-up scheduling, and the transition back to the home environment. These guides draw on general best practices and your care recipient's specific profile.
Viewing event history and patterns
The acute events timeline shows all logged events in chronological order. You can filter by event type to see, for example, all falls over the past year. If the pattern analysis system detects a concerning trend, such as increased falls or recurring infections, it may generate an alert suggesting you discuss the pattern with the primary care provider. This history is also included in care reports and can be printed or exported for doctor visits.
